During an inquest into the tragic stabbing incident at Bondi Junction Westfield on April 13, 2024, it was revealed that the assailant, Joel Cauchi, had a fixation on weapons and violence prior to the attack. Cauchi, who had been diagnosed with schizophrenia in his youth, fell through the cracks of the mental health care system and had not received treatment for five years before committing the horrific act.

On that fateful day, Cauchi, armed with a knife, launched an indiscriminate assault in the bustling shopping centre, resulting in the deaths of six individuals, including a security guard, and injuries to another ten, one of whom was a nine-month-old girl. The police shot and killed Cauchi at the scene.

Coroner Teresa O’Sullivan presided over the inquest, during which counsel assisting, Peggy Dwyer SC, presented evidence of Cauchi’s troubling mental state. Dwyer described his internet search history—focused on weapons, violence, and notorious mass killings—as deeply distressing. Among the searches were inquiries about high-profile cases, including the Columbine High School shooting.

The inquest highlighted that Cauchi’s mental health struggles went largely unmonitored after he moved from Queensland, where his parents had previously managed his condition. Notes on his phone indicated plans to carry out an attack at a shopping centre, underscoring his severe mental health decline. Expert psychiatric testimony during the inquest confirmed that Cauchi was “floridly psychotic” at the time of the incident.

The coroner’s examination will extend to the interactions Cauchi had with police and any lapses in his mental health care that may have contributed to the tragedy. Dwyer noted that security personnel and media responses to the incident would also be scrutinised.

The six victims—Dawn Singleton, Ashlee Good, Jade Young, Pikria Darchia, Yixuan Cheng, and Faraz Tahir—were memorialised at a tribute site, highlighting the community’s ongoing grief following the attack. The inquest aims to uncover actionable insights and recommendations to prevent similar occurrences in the future.
