Father’s Death in Perth Factory Deemed a ‘Tragic Incident That Could Have Been Prevented’

Unions are expressing outrage over the tragic death of a 45-year-old apprentice at Hofmann Engineering’s Bassendean factory in Perth, describing it as a “preventable tragedy.” The incident marks the second fatality at the facility in a six-year period, with work carrying on at the site following the accident.

The victim, who had recently embarked on a trade career change from being a chef, was attempting to wrap a six-metre forging ring in plastic when it slipped and fatally crushed him. He had moved to Australia two years prior and had since gained permanent residency. Described as a dedicated father to his 10-year-old daughter, he took on the job to spend more quality time with his family.

Steve McCartney, the state secretary of the Australian Manufacturing Workers’ Union, highlighted the dangerous circumstances, stating that “this is a three-tonne bit of equipment that was put in the wrong place at the wrong time.” Following the tragic event, the man’s colleagues returned to work and were provided with counselling services to support them during this difficult time.

The incident has raised alarm, especially given a previous fatal accident at the same site, where a 46-year-old man was killed six years ago, resulting in a fine for the company. WorkSafe has expressed concerns over the repeated incidents at the Bassendean site, prompting inquiries into workplace safety protocols.

WA Premier Roger Cook emphasized that all employers have a fundamental responsibility to ensure the safety of their workers. In a related note, Hofmann Engineering had recently received state government funding aimed at securing overseas projects, underscoring the focus on growth despite serious safety challenges at their facilities.
